Pure Health Foundational Assessment
One-Time Assessment with the Pure Health Team
A deep dive consultation where you learn about the unique imbalances that are impacting your symptoms and disease. This assessment gives you a detailed picture of your current state of health. Together we will develop a strategy for optimal living personalized for you.
As we conclude our visit together, you will be provided recommendations on nutrition, sleep, exercise, stress, baseline supplements based on labs reviewed, and any additional recommendations and follow-up.
This is the first step in establishing care with us and to start our 12-Month Membership Program. However, there is NO obligation to proceed with membership.
90-minute comprehensive visit with a Pure Health Provider (NP/PA)
One-time investment of $485 includes items listed here.
We will conduct a thorough lab review and discuss with you any nutritional deficiencies, hormone imbalances, inflammation and more.
Review of advanced functional lab work with an in-depth diagnostic panel
*Cost of labs is in addition to Pure Health Foundational Assessment.
A Health Coach session & Athletic Trainer session
These 2 sessions are included to help you move into action.
Evaluation with InBody Dial H20 Smart Body Composition Scale
Measures whole-body composition to allow for targeted and personalized plan of care and track meaningful changes and results.
2 infrared sauna therapy sessions, with chromotherapy
Pure Health Membership Plan
12-Month Program to continue working with the Pure Health Team receiving our benefits and services
A year long program to continue the deep dive into your health to address imbalances at the root-cause.
*Your Foundational Assessment must first be completed prior to entering our Membership Plan. During the Pure Health Foundational Assessment, you will discuss with your provider if we are a good fit for each other and if you would benefit from our membership. Completion of the Pure Health Foundational Assessment does NOT guarantee a spot in our Membership Plan.
