Foundational Assessment

We work with you to determine the root cause of your symptoms and imbalances.
We will partner with you as we develop a compressive plan individualized for you.
We take a patient-centered holistic approach to healing.

This assessment is the first step in joining our year long membership,

however there is no obligation to proceed with membership.

Step 1: Lab Draw

Your first step is to schedule an appointment with Pure Health to come in for a lab draw. These are advanced labs will look at nutritional deficiencies, inflammation and more.
We will review these results with you at your assessment with your provider. The cost of the labs is a separate charge. See our FAQs page for more info.
Step 2: Assessment with Pure Health Provider

Once we have received your lab results, we will invite you to schedule your appointment with a Pure Health Provider (NP/PA). This is a 90-minute in-person visit that takes a deep-dive into your current health concerns and provides you with a comprehensive plan. You will feel heard and have a clear understanding of your plan moving forward.
Step 3: Pure Health Team Support

The Foundational Assessment includes 3 visits. After meeting with your provider, you will schedule an appointment with our health coach and an appointment with our athletic trainer. These two additional sessions are to help you take your plan and move into action.
Next Step: Consider Our Membership Program

After completion of your assessment, your provider with discuss if the membership program could be of benefit to you. We will discuss with you what advanced testing and team support we offer in the year long membership. This is where the deep work occurs.
Completion of the Foundational Assessment does not guarantee a spot in our membership plan.
